An OatWizard in three steps. Next validates the step's fields; the numbers of finished steps take you back. The customer is looked up on the server as you type (OatAutoCompleteField), colleagues to notify are chosen as chips (OatMultiSelectField), and the delivery window is an OatDateRangeField.
